Sport Physical Forms for 2009-2010 Vanderbilt Sports Medicine is offering $5 physicals on Saturday, May 2 for our students at 1:15pm. Students will need to have the paperwork filled out with a parent’s signature. The applications for the physicals can be picked up from coaches. This physical will be good for the school year 2009-2010. All athletes must have a physical each year. If they do not get a physical from Vanderbilt, then they will need one from their primary care doctor or a walk-in clinic dated after May 1, 2009.
